Letter From Immunisation Service HPV Vaccine

Dear Parents/Guardians
All year 8 children are being offered the HPV vaccination which protects against a range of HPV related cancers.
This is a two dose programme and by completing the consent form you are consenting to both doses.
Can we kindly ask if you do not wish your child to receive this vaccine would you complete the consent form indicating your decline, to avoid any further follow up correspondence from our service.
